Three Sip Society, LLC – Tasting Experiences

By completing this Participant Waiver Form below, you acknowledge and agree to the following:

1. Assumption of Risk
   I understand that participation in a tasting experience offered by Three Sip Society, LLC (“Company”) may involve the consumption of alcoholic beverages. I acknowledge the inherent risks associated with alcohol consumption, including but not limited to intoxication, impaired judgment, allergic reaction, or interaction with medication. I voluntarily assume full responsibility for any risk of accident, injury, illness, or damages arising from my participation.

2. Tasting Fee
   I understand and agree that the fee paid for any tasting event is solely for educational and experiential services. Company does not sell or distribute alcohol. All alcohol is purchased separately by the host of the event from a licensed retailer.

3. Compliance with Law
   I affirm that I am of legal drinking age (21+) and will consume alcohol responsibly in compliance with applicable laws. I will not operate a motor vehicle or engage in unsafe activities after consuming alcohol.

4. Release and Waiver
   To the fullest extent permitted by law, I release and discharge the Company, its owners, officers, employees, contractors, and agents from any and all liability, claims, or causes of action arising from my participation, whether caused by negligence or otherwise.

5. Indemnification
   I agree to indemnify and hold harmless the Company against any losses, liabilities, damages, or expenses (including attorney’s fees) arising out of my participation.

6. Medical Disclaimer
   I understand that individuals with certain medical conditions (e.g., pregnancy, high blood pressure, medication use) may be at increased risk. I voluntarily assume responsibility for participation.
